Please be aware that the Compliance Questionnaires and Personnel Certifications are now housed within Fibi.
After starting a record, the Compliance Questionnaires can be accessed in two different ways:
1. PIs and Key Personnel can complete their certifications and questionnaires in Fibi.
2. Administrators can print a fillable version of the questionnaires to then share.
Please do not use older forms as many of the questions have changed or been reordered.
